Goal Attainment Scaling (GAS) at Quad Care: Measuring Meaningful Progress in Occupational Therapy
Goal Attainment Scaling (GAS) is a structured, evidence‑based outcome measure that allows occupational therapists to capture these highly individual achievements in a way that is meaningful, measurable, and NDIS‑aligned.
In occupational therapy, progress is not one‑size‑fits‑all. What represents a major achievement for one person may be a small step for another. For an adult recovering from stroke, independently pouring a cup of tea may be life‑changing. For a child with autism, remaining regulated in a busy classroom for ten minutes can be a significant milestone.
Quad Care’s Occupational Therapy team uses Goal Attainment Scaling to support participant‑led goal setting, track functional progress, and clearly demonstrate outcomes for NDIS planning, reviews, and progress reporting.
What Is Goal Attainment Scaling (GAS)?
Goal Attainment Scaling (GAS) is an individualised outcome measurement framework used in occupational therapy and other allied health disciplines. Instead of relying on standardised norms, GAS measures success against person‑specific goals that reflect what matters most to the participant.
Each goal is scored on a 5‑point scale, allowing therapists and participants to capture progress that may fall below, meet, or exceed expectations.
This makes GAS particularly useful in NDIS‑funded therapy, where goals are highly individual and outcomes need to reflect real‑world participation rather than standard benchmarks.
Why Quad Care Uses Goal Attainment Scaling
The NDIS places strong emphasis on:
- participant choice and control
- functional impact
- evidence of progress over time
- participant safety and safeguarding
Goal Attainment Scaling aligns closely with these principles. At Quad Care, we use GAS because it:
- supports participant‑led goal setting
- recognises incremental progress, not just pass/fail outcomes
- provides clear, observable evidence of change
- supports transparent, defensible NDIS reporting
GAS helps our Occupational Therapists show not just whether progress occurred, but how it occurred in everyday life.

How Goal Attainment Scaling Works
Each GAS goal is scored using a five‑point scale, typically ranging from –2 to +2:
- –2: Baseline performance (much less than expected outcome)
- –1: Some progress, but less than expected
- 0: Expected outcome (the agreed therapy goal)
- +1: More than expected progress
- +2: Much more than expected progress
The 0 score represents the realistic, agreed outcome for the end of the intervention period. Progress above or below this level is still captured and acknowledged, which is especially important in complex or fluctuating conditions.
Writing Effective GAS Goals
The quality of Goal Attainment Scaling depends on how clearly the goal levels are written. At Quad Care, our OTs ensure GAS goals are:
- Observable – based on what can be seen or measured
- Measurable – using time, frequency, assistance level, or prompts
- Mutually exclusive – each level is clearly distinct
- Focused on one variable – to avoid confusion when scoring
For example, instead of stating “improved dressing skills,” a GAS goal may describe:
- level of assistance required
- number of steps completed independently
- time taken to complete a task
This clarity supports consistent scoring and meaningful outcome tracking.
Goal Attainment Scaling and the NDIS
NDIS reviews and plan decisions require clear evidence of functional impact and progress. GAS supports this by:
- establishing a participant‑specific baseline
- tracking progress in real‑world tasks
- demonstrating outcomes linked directly to NDIS goals
At Quad Care, GAS may be used alongside other assessments and clinical observations to support:
- NDIS progress reports
- plan reviews
- capacity‑building documentation
Use of Goal Attainment Scaling supports evidence‑based reporting but does not guarantee NDIS funding outcomes. All funding decisions are made by the NDIA based on individual circumstances.
Using GAS Across Different Populations
Goal Attainment Scaling is highly adaptable and can be used across the lifespan.
Children and Families
GAS supports collaboration with families to focus therapy on daily routines such as dressing, mealtimes, school participation, or emotional regulation.
Adults and Neurorehabilitation
For people recovering from stroke, brain injury, or neurological conditions, GAS captures gradual improvements while acknowledging day‑to‑day variability.
Education and Community Settings
Because GAS goals are written in clear, observable terms, teachers, support workers, and family members can help monitor progress outside therapy sessions.
This flexibility makes GAS a powerful tool for community‑based occupational therapy, which is a key focus at Quad Care.
Goal Attainment Scaling and the COPM: Working Together
Goal Attainment Scaling is often used alongside the Canadian Occupational Performance Measure (COPM).
- COPM identifies what matters most to the participant
- GAS defines how progress toward those priorities will be measured
Together, these tools represent best practice in client‑centred, outcome‑focused occupational therapy.
Benefits of Goal Attainment Scaling
Using GAS in occupational therapy offers several advantages:
- Validates small but meaningful progress
- Improves participant motivation and engagement
- Supports clear communication with families and support teams
- Provides strong outcome data for NDIS reporting
- Aligns clinical goals with everyday participation
These benefits align closely with Quad Care’s commitment to practical, person‑centred therapy.
